Group Adult Foster Care (GAFC):

We aid members with Activities of Daily Living (ADLs) such as assistance with mobility in bed and transferring, ambulation both in and out of the home, help with getting dressed, eating, toileting, bathing as well as personal hygiene.

We also support Instrumental Activities of Daily Living (IADLs) such as helping prepare meals, conducting light housework, managing medication, phone use, assisting with grocery shopping, and transporting to medical appointments.

Managed Care

Provides more daily flexibility with a wider range of hours that are dependent on a member’s condition. An interprofessional team is provided to decrease hospital admissions through increased supervision, a variety of programs to attend all while allowing members to stay in their homes and communities. The program serves to provide companionship, home making and personal care for those over 65 as part of the SCO program as well as individuals between the ages of 21 and 64 who are dually eligible for both Medicare and Medicaid.

Am I eligible for GAFC?

To enroll in Group Adult Foster Care, you must:

Be at least 22 years of age
Be eligible for MassHealth Standard
Live in an assisted living facility or other housing that participates in GAFC
Require daily assistance with at least one Activity of Daily Living (ADL), which is related to a medical diagnosis
Have had a physical by your primary care physician (PCP) within the past 12 months
